Sliding Fee Discount Program
We offer services on a sliding fee scale based on income and family size to ensure affordability for all. No one will be denied access to services due to inability to pay. The discount is based in household income and size, and patients must provide proof of income. To qualify for sliding fee scale services you must bring one of the following documents:
•  Latest Income Tax Return
•  Previous year W2
•  One month of paystubs or notarized letter of employment from your employer

Homestead Community Health Center in Miami Dade county, Florida provides high quality, comprehensive, primary and preventive medical, and behavioral health care services and education in an environment of caring, respect and dignity. Homestead Community Health Center serves patients in the residential and commercial Miami Dade / Homestead area.
Our medical providers are highly trained and board certified from many diverse, cross-cultural backgrounds. The care and service they provide is done so in a respectful and comfortable environment where dignity and understanding allow for the patient/client and provider to share information.
A multidisciplinary team works together to meet most patient needs through the provision of evidence-based, patient-centered, easily accessible comprehensive care and coordinates with other specialties as necessary.
